CLHStat Terms and Conditions

Clinical Labs of Hawaii (CLH) is pleased to be able to provide you access to patient laboratory results via our mobile device application, CLHStat. Before accessing your account, please review the Terms and Conditions set forth below.

General Terms

By clicking "Agree and Continue", I am agreeing to the following terms and conditions:
I am requesting access to personal health information for patients in my care. I understand the access is provided over the Internet using an electronic application ("App") called CLHStat, (available for download via the iTunes App Store or Google Play). I hereby request CLH to disclose identifiable health information to the designated App account for the purposes described below. My continued use of the App indicates my agreement to abide by these terms and conditions.
I understand that my use of the App is voluntary and that I may cancel the account at any time. I understand if I decide at any time to cancel the account it will not affect my status as a client of CLH.

Verification of Identity

I understand that creating the App account is contingent upon verification of my identity by CLH or Experian Information Systems ("Experian") on behalf of CLH.

App Activation and Information Security

Upon completion of my identity verification I will be prompted to establish my CLH account username (unique email address) and password. After installing the application I will provide my CLH account username, enter the corresponding password, and establish a PIN to activate the CLHStat app. I understand my CLH acount username, password, and PIN are unique items that identify me in the App and allow me to access a patient's personal health information.
The CLH account username, password, and PIN provide multiple factors of authentication. CLH stores this information in a secure database that is isolated from the Internet. I understand I am responsible for: 1) keeping my username, password, and PIN confidential; and 2) changing my password and PIN if, at any time, I think the confidentiality of my password or PIN may have been compromised.
I understand the App will lock when it is put in the background or after fifteen (15) minutes of inactivity. My App account will automatically be disabled after seven (7) failed log-in attempts. I understand I may contact CLH Support for assistance. See the System Requirements and Technical Assistance section below for support contact information.
I understand that CLH accepts no responsibility for and disclaims any and all liability or consequential damages arising from a breach of health information confidentiality resulting from my sharing or losing my password or PIN. If CLH discovers that I have inappropriately shared my password and/or PIN with another person, or that I have misused or abused access privileges in any way, CLH may discontinue my participation in the App without prior notice.
The information in the Portal is protected using encryption technology.

Access Termination

I understand I can terminate my App access at any time by submitting a written request to CLH. I also understand my App account access will be disabled after 12 months of account inactivity.

Copyright and Trademarks

CLHStat is a registered trademark of Clinical Laboratories of Hawaii, LLP. All content included in the App, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions. The Content includes both Content owned or controlled by CLH and Content owned or controlled by third parties and licensed to CLH. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of CLH.

Site Access and Licenses

CLH grants a limited license to each App user for personal use only of the App and the associated services in accordance with these terms and conditions of use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the App and the associated services, making any derivative of the App or the associated services.


I understand patient privacy is of utmost importance to CLH. The creation of a Portal account and its use is consistent with the CLH Notice of Privacy Practices. I may review the CLH Notice of Privacy Practices via the Privacy link on the Portal website or at for a thorough description of how personal health information is collected, utilized, and protected.

System Requirements and Technical Assistance

I understand the App is supported by Apple iOS and Android. I understand CLH provides limited technical assistance for App users. Support can be contacted for assistance at (808) 677-7998.


The App is not designed to make diagnoses. Only a healthcare provider can properly diagnose and treat illness or injury.
I understand that all health-related information provided through the App is intended to assist the healthcare provider and should not be used as a substitute for the provider's own judgement or determination regarding diagnosis and/or patient care. CLH is not responsible for how I use the information obtained.
The App may not be available to users all the time. Access is provided on an as-is, as-available basis. CLH does not guarantee users will be able to access the App at any particular time due to system failures, back-up procedures, maintenance, or other causes beyond the control of CLH. CLH reserves the right to change the features of the App and this Terms and Conditions agreement at any time. The current Terms and Conditions agreement can be viewed at any time by clicking on the Terms and Conditions link in the App.
